If you want to fill out your pre-study questionnaires and other forms before arriving at the sleep center, you can download below, then print, read, and complete. Please bring your forms to your sleep study appointment and submit them to the overnight sleep technician.

Pediatric Forms

For parents of pediatric patients: Please download, print, and complete the pediatric sleep history questionnaire (below) instead of the regular sleep history questionnaire (above). Please also fill out the Patient Registration form and sign the Consent form, HIPAA Privacy Notice, and Payment Policy (above).

Please note that the time alotted for filling out forms after your arrival to the sleep center may not be enough time for you to carefully read and meticulously fill out these required forms. (Please see approximate timeline)  

It is strongly encouraged that you read and fill out forms before you arrive at the sleep center for procedural efficiency. If you do not have access to a printer, we may be able to mail or fax the forms to you; please contact us to make arrangements if needed, or if you have any further questions or concerns.
